Trunk-to-Trunk calls

I have configured my Trixbox CE 2.6.2.2 with two SIP trunks one that goes to my VOIP provider and a second that goes to my SIP gateway.
I have X-lite clients registered with my Trixbox and can make calls between them.
I can make and receive calls from my X-lite clients to a phone on the PSTN.
I can make calls between my SIP gateway and the X-lite clients
I am having problems making calls into and out my SIP gateway to/from the PSTN
I configured my SIP trunk for my SIP gateway as follows...

host=10.11.12.93
username=sip-gateway
secret=sip-gateway
type=peer
context=from-internal
canreinvite=yes

When i make a call from a pstn user my IVR answers I then press an extension that i configured for that trunk but I get a message that says it is not a valid extension.
How do i route inbound calls that begin with extension 41xxx to the correct SIP trunk. PSTN--->Trixbox---->SIP Trunk
My next question is how do i route calls from my SIP trunk out to my SIP trunk that goes to my VOIP provider. SIP Trunk---->Trixbox---->VOIP provider (SIP Trunk)

Have a look at the DISA module this allows you to call into your trixbox server and get a dialtone and call out on another trunk. However, it does let outside users run up a large bill for you unless you restrict access. So you need to think about who you open this up to. There have been several threads here on DISA.
