CID not working, inbound calls ringing twice, hanging up

I upgraded a 2.4 system to a 2.6 with a new server, same TDM01b and Polycom phones. The previous system had a few glitches but incoming CID and calls worked. Now all of my calls are coming in as Unknown Unknown and don't ring long enough despite changing ring time settings. I've been trying to figure this thing out for a couple weeks now, and my client's not too pleased. Any help would be very much appreciated. Below are links to my log and relevant conf files:

I think i may have narrowed

I think i may have narrowed it down to a problem in one or two of my FXO modules. One module works fine, passing on CID, etc. The other two don't do CID and cut calls off prematurely. Are there any diagnostics i could run to see if my modules are ok?

This is identical to a problem I'm having, and I just had somebody else tell me that it sounded like a hardware problem.

Did you figure out how to check the card and modules?

His problem as sounds

His problem as sounds familiar to one of my locations, who seem to have the same problems, but more sporadically.
