Caller ID and Misc Dest

Certain Sip Providers require your caller id to complete the outbound call---But if you use Misc Destination the caller id of the person thats calling you is presented to the Sip provider and the call is rejected----Any Ideas on how to associate your caller id with Misc Destinations Not counting building a Trunk for every user????
